Biblio and item level detail need to be surrounded by opening and closing
<item> tags:

<item>
Title: <<biblio.title>>,  by <<biblio.author>>
Accession No:  <<items.barcode>>
Check out (Borrowed) date:  <<issues.issuedate>>
Due Date:  <<issues.date_due>>
