[Koha] Error- 404 - The requested URL / was not found on this server.

It's a bug in the 3.16 packages when running under apache 2.2. All it
causes to happen is that it fails to re-enable the site after the
change.

You can fix this by running:

sudo a2ensite library.conf

(where library is your instance name.)

Then:

sudo service apache2 restart
