Did you set the OPAC system preferences related to self registration? Check 
out Home -> Koha administration -> Global system preferences -> OPAC and 
then scroll down to the "Self Registration". You are probably missing 
PatronSelfRegistrationDefaultCategory set to a value found in Home -> Koha 
adminsitration -> Patron categories.
