[Koha] Yet another request of changing password and few small others.

Hi all,

I work for a small company and I was asked to take care of some
administrative tasks in Koha.
I have to admit that I'm pretty new to Koha and I have some questions.

I inherited the installation and the mysql database from the previous
person who administered the DB.
The authentication is against the AD and as to itself it works fine
although we get an error on the fact that for Koha is not possible to add
new members

"AddMember failed at /usr/share/koha/lib/C4/Auth_with_ldap_pm" at line 169

I can access the mysql db without any problem and in the DB I see there are
2 users, (who left the company) with, I guess, the hash of their passwords
since I don't think passwords are stored in clear.

Now we have new books to add but basically no valid user to access the DB.

Here my questions:

1) if the authentication fails against the Active Directory, is the
fallback to local DB automatic or do I have to conigure somewhere?
2) how can I reset the password of any of those two users in the mysql DB?
I don't have access to the Koha web interface;
3) is there a way to administer local users via the mysql interface? Would
be good to add a new user while troubleshooting why the add of AD users
doesn't work sowe can dd new books into the system.
4) how I can add a Koha administrator account via the mysql interface?

I have to admit that I'm bit in a hurry and I tried to through the manual
but I felt a bit lost in gathering the right information to solve my
problems I wrote above.

If someone could help me in recovering this situation or pointing me in the
right direction to recover it that will be much appreciated.
