[Koha] LIS professionals participation and contribution to Koha community: a survey

I'd like to 3rd that thought...
If this survey is for publication, information on
attribution/anonymity rules would be much appreciated.  Informed
consent, etc parameters would be
nice as even a survey is usually considered human subjects research!
