[Koha] Koha Itemtypes Unavailable and Not Showing in Intranet or Opac

I have had two library instances running
for several years but I have never been
able to get the itemtype images to show
up in either intranet or opac since
upgrading to 3.x.

I have scoured the files and cannot figure out the problem.

When I go to Global Administration in intranet
and to Item types, whether trying to enter a
new item type or edit an existing item type,
the only tabs that are displayed are "None" 
and "Remote Image."

None of the imagetype libraries are available
to select.  Does anyone have any ideas what 
the problem might be here?  Oh, and Yes, 
showItemtypeImg is set to "Show." When you 
hover over the itemtype image location in Opac,
it shows what the itemtype is (e.g., book)
but no image is displayed.

Running Koha 3.08.06.000 on
up-to-date Debian Squeeze (x64).
