[Koha] Koha 3.02.00.004 to 3.06.3 database upgrade fails

Hi,

I tried upgrading database created under Koha version 3.02.00.004 to
version 3.06.3 and I get the following errors.

Update errors :

    * [Fri Feb 24 11:52:26 2012] updatedatabase.pl: DBD::mysql::db do
failed: Duplicate column name 'privacy' at
/usr/share/koha/intranet/cgi-bin/installer/data/mysql/updatedatabase.pl
line 3997.
    * [Fri Feb 24 11:52:26 2012] updatedatabase.pl: DBD::mysql::db do
failed: Duplicate column name 'privacy' at
/usr/share/koha/intranet/cgi-bin/installer/data/mysql/updatedatabase.pl

After database upgrade is done, I get the following error in web browser:

Software error:

Can't call method "delete" on an undefined value at
/usr/share/koha/lib/C4/Auth.pm line 717.

...

Any tips to solve this issues would be greatly appreciated. Thanks!
