[Koha] Circulation problem

Dear all

 

We are using koha 3.0.6, facing problem in circulation module that patron
has returned the book and when i have reissued same book to the same patron
, it's  not checked out. A box, top of the message box displayed but message
not appeared. No other books are allowed to issued in same patron and no
message are displaying, only small box appeared. I have issued same book to
other patron in same category with same status of fine, this item issued.
The screen shot are attached.

 

Kindly help me.
