[Koha] Label printing empty

Dear All,

I have problem to configure the label printer of TSC TTP342E pro for koha.
Please see koha printer configuration in attached file:
filename:koha-template.

When i create PDF format all labels empty :filename. label_batch_9.pdf

if I create CSV format file, i have barcode looks like in attached
file:koha-level.csv

What is wrong for my configuration. My label is 40 x 20 mm ribbon.

I am looking forward any help.
