Actually that part of both the on screen and other documentation needs some
help. Several of us have discussed this for a while now, but nobody's had
time to do anything about it.

For starters, your best bet is to download the sample csv and build from
there. The error you posted indicates that the header row (or row 1 in your
spreadsheet) is malformed.

If you are using extended patron attributes, you may want to read this
comment: http://bugs.koha-community.org/bugzilla3/show_bug.cgi?id=5379#c14regarding
proper syntax.
