[Koha] Patron Card label not created

Dear all

i am trying to create patron card with barcode and insert the cardnumber,
surname, firstname, title, streetnumber, address, city, zipcode in data fild

Label Layout) option in lable layout  Then  try to generate lable but patron
lable is not generted only white pdf file is opened.

Please guide how to create patron lable step by step.

 I have installed koha ver.3.00.02.012 with Ubuntu
