[Koha] How to solve this error?

Dear Friends,
We encountered an error while cataloging, saving item information with Koha
3.03 on CentOS 5.3.
I request your help to solve this problem.
I already posted the same two days back, but nobody replied. We tried a lot
to solve the problem, we have not succeeded yet.
