[Koha] Error in adding item

Hello
While adding item after adding catalog, it is giving following error.

Error that we are getting is.
Can't call method "append_fields" on an undefined value at
/usr/lib/perl5/site_perl/5.8.8/MARC/File/SAX.pm line 92. at
/usr/lib/perl5/site_perl/5.8.8/i386-linux-thread-multi/XML/LibXML/SAX.pm
line 80 at /usr/lib/perl5/site_perl/5.8.8/MARC/File/XML.pm line 445
