[Koha] Migration from Millennium/III to Koha

As far as I can tell, this has never been done before. 

 

My library is exploring this switch, as well.  I'm the primary on this
project, and I'm currently working on the data migration portion of the
job.  I plan to publish all my findings and methods once I've got them
figured out.  With any luck, that will be of some use to other III shops
looking to migrate.
