[Koha] Issue with the Issues Table

I have a problem charging items to patrons.  While the items table is updated properly when items are checkedout, the issues table is not affected.  It seems to be a foreign key issue.  This is on Koha 3.0.1.  Here is the error message:

Has anybody encountered this?  Is it a bug?  Thanks.
