[Koha] borrower state

This would be an enhancement request - one that I completely agree
with.  I don't know why City State were in one field - maybe cause all
countries don't have city/state designations - but that doesn't mean
we can't have an extra field that some people don't use.

Now all we need is someone to do the development - and a sponsor.
