[Koha] Error in authorities module of Koha 2.2.9

Hi all,

I have problem in 'authorities' module of Koha 2.2.9(Windows Version).I
cannot 'add authority' due to some problem in the form 'authorities.pl'.Also
I cannot import member pictures in member information.
