[Koha] Circulation status in moredtail.pl

In an upgrade from 2.2.5 to version 3, I'm having problems with the 
Items tab in mordetail.  Items are listed with this circulation status.

Circulation Status: Checked out to , Due back on 00/02/0

Here are the errors in koha-error_log:

I also have no options in the drop-down menus for Lost Status and 
Damaged Status.  I filled in authorized values for LOST, but not DAMAGED 
since we don't have any items marked damaged int he current system yet.
