Ok, what version of Koha and what version of MySQL are you running, I  
suspect its to do with date formats.

Barcode image? Not sure what you mean here, if you could do a  
screenshot for me off list.

Sure can, easy way is to edit the template to remove the blank entry,  
then the first one will be your only branch
