[Koha] Guidelines for Koha Installation (Debian)

Check out the Mysql manual at http://dev.mysql.com/doc/refman/5.0/en/
The link is for version 5 but links to other versions are also listed.
The default username is root without a password. The manual shows how to
enable a root password as well as create users with password access to
the database.
