Have you been through the update database process? Did you notice an error when you upgraded? You have a missing table. I would suggest to use a backup of your database, restart memcached then koha-common and hit the intranet page. You should then see the upgrade page.

If none of the above worked (or if you don't have backup), try to create the missing DB columns manually:

After some troubleshooting, I found out the error "*Internal Server Error*" is triggered when plack is enabled. After the disabling of plack, I managed to view the staff login page with an error message below;
