The manual has a section on this: http://koha-community.org/documentation/3-2-manual/?ch=c18#AEN1230
Also note that the images aren't pulled into Koha when you add the book--they're not pulled into Koha at all. They're displayed dynamically when you view the record. The images reside on the server of whichever service you're using, Amazon, Google, etc.

That's right. They don't cooperate. Development is being planned, I believe, to make the book cover options play nicer together: having Amazon fail over to Google if a cover isn't found, for instance.
