Ooops. Please delete line 138. It should have been deleted. Was this a CVS copy? There's a big fix going on in there at the moment and I probably should be using a staging area instead of CVS HEAD. I didn't realise at the outset what a huge fix this was going to be.

Sounds like the pre5 includes path bug. Please make sure that your koha.conf includes= line points to where your includes files really are.
