I think the large public libraries and consortia in the U.S. now switching to Koha would disagree with that characterization!
But I don't think they use Koha as I can donwload it from internet. They have a paid support or a good tecnical staff (IT and librarian) to manipolate at least templates and a to change the thing at proper level. For example: I need to swith the Opac from an english monolingual to a multilingual with Italian and English, default in Italian. To do it I need to do a SQL update because the system prefences lists languages in alphabetic list, so English is before of Italian. No problem if you have a monolingual Opac but multilingual opac are a must for Italian university libraries. In fact with enough support you can use Koha for every type and size of library, I think.
