If you are using Apache authentication then you have to maintain the passwords independently of Koha. The passwords are stored in /etc/kohaintranet.pass You can add/modify this file with the htpasswd utility that should come with apache: htpasswd /etc/kohaintranet.pass jsmith will prompt for a password for the jsmith user.
