By default, Koha allows 3 text fields, 2 images (one of which is usually the patron image) and 1 barcode on a card. If you need more than that you have to read up the related Perl code and dabble with XML. There are no fixed sizes as different libraries use different size of card. CR80 is one of the most popular sizes for ID cards (same size a Credit / ATM card). Figure out what size you require, sit down with a straight edge rule and a calculator, along with Koha's patron card creator tool and work it out.
