[Koha] First association/foundation poll results

Survey summary information:

1954 responses for this survey (1592 full responses, 362 responses not
completely filled out)

Of the full responses, I have yet to clean out junk responses - but I
have the raw results and a PDF of the charts of results available - I
am putting them on the wiki as we speak - since I don't want to send
out large attachments to the mailing list.  The PDF only shows
completed responses - and the raw results are just that - all results.
