I suspect this one is related to your problem:

with no item throws error

Workaround: If you fill out the item type field it should save without
an error and an item is created at the record you are ordering.
To avoid creation of items in acquisitions you can set the sys
pref AcqCreateItem to ‘cataloging’.

Subject: [Koha] Acquisition module Error 500
Dear Friends,
I working with Koha 3.02 Acquisition module.
I created a vendor and basket.
I got the error message " An error has occured. Error 500" when i
added a new order for books.
I have not filled the item information (barcode and related information) in the
order space. But mew order placed.
When i entered item information for another new order, two order for same book
created.
Whats the wrong with acquisition module?
